Register your child/children (ages 0–5) for the 10:30 AM Fall Mini Session of Lil’ Music Makers. Siblings under 8 months are free but must be registered. Be sure to read, complete and sign the Lil’ Music Makers Registration Agreement. Then you’ll be directed to the payment page to complete checkout.
Lil’ Music Makers Registration Agreement
By registering, parent/guardian acknowledges and agrees to the following:
Class Info & Policies
Guests: Adult family and friends are welcome. Children 6+ may attend if they participate and stay with you; please email Cheryl in advance.
Adult Participation: Join in and have fun! Model sitting and movement; take a break when needed.
Safety: Children must always be supervised; running without guidance is not allowed.
Illness: Keep children home if contagious.
Missed Classes & Cancellations: No credits for missed classes. Makeup classes may be offered if space allows. Email Cheryl if absent. No refunds for weather-related cancellations. If Cheryl cancels, a make-up may be offered if possible.
Refunds: Full refunds are available if requested before the second week of classes. After that, partial refunds may be considered case by case. Refunds typically appear within 5–10 business days.
Release of Liability:
I understand there are risks of physical injury in participating in recreational activities or programs.
I hereby release, waive, discharge, and agree to indemnify and hold harmless Cheryl Mandracchia of Music Time with Miss Cheryl LLC, the First Congregational Church of New Milford, the Marsh Parish House (connected to the Church), and their employees, officials, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or other casualty of any kind that may occur to my child(ren), myself, or my child(ren)’s caregiver(s) while participating in Lil’ Music Makers classes or any related activities.
Medical Consent:
I consent to emergency medical care for my child if I or designated caregiver cannot be reached. Music Time with Miss Cheryl LLC does not provide accident or hospitalization insurance; participants should have personal coverage and consider their own health, experience, and risk tolerance before participating.
Privacy / Data Collection
Music Time with Miss Cheryl LLC collects: child’s name and age/birth month & year, parent/guardian contact, payment info (via Stripe), and optional details (e.g., allergies). Used for registration, class updates, payments, and rosters. Stored securely, shared only for payments or as required by law. Parental consent is required for children under 13 (U.S.) or 16 (EU). Parents/guardians can review, update, or request deletion. Collection follows COPPA and GDPR rules.
