If YES: You (the parent/legal guardian listed on this registration form) give permission for Music Time with Miss Cheryl to use photos/videos of your child(ren) for marketing purposes. No names or identifying information will be shared. You understand that no compensation will be provided for the use of these images. The date of this form submission will be recorded.

If NO: You do not grant permission for photos/videos of your child(ren) to be used.

Lil’ Music Makers Registration Agreement — 2026
(Includes Liability Waiver, class policies and additional agreements)

By registering, parent/guardian acknowledges and agrees to the following:

Communication:

Families agree to regularly check their email for important Lil’ Music Makers class updates, notices, and shared information. Email is the primary method of communication.

Class Info & Policies

Guests: Adult family and friends are welcome. Children 6+ may attend if they participate and stay with you; please email Cheryl in advance.

Adult Participation: Join in and have fun! Model sitting and movement; take a break when needed.

Safety: Children must always be supervised; running without guidance is not allowed.

Illness: Keep children home if contagious.

Missed Classes & Cancellations

There are no refunds for missed classes. If your child will be absent, please email Miss Cheryl. Makeup classes may be offered if space allows but are not guaranteed. Classes canceled due to weather are non-refundable. If Miss Cheryl must cancel a class, a makeup class may be offered when possible.

Refunds:

Full-Term Classes (8-Week Sessions)
Full refunds may be requested prior to the second week of class. After the second week, refunds are not guaranteed and may be considered on a case-by-case basis.

4-Week Half Sessions (First or Second Half)

Cancellations requested before the first class of the selected half session may receive a full refund, minus a $15 administrative/materials fee.

Once the half session begins, no refunds are issued for missed classes or withdrawals, as spots are limited and difficult to refill.

If Miss Cheryl must cancel a class, a make-up class or credit will be offered when possible.

Single-Day Events

Cancellations made 24+ hours in advance receive a full refund or credit. Cancellations within 24 hours or no-shows are non-refundable; however, a credit for a future single-day event may be issued upon request, at instructor discretion and subject to availability. To be considered for a credit, Miss Cheryl must be notified by email prior to the event. Refunds are typically processed within 5–10 business days.

Liability Waiver:

I understand that participation in Lil’ Music Makers involves inherent risks. I assume full responsibility for these risks and release and hold harmless Cheryl Mandracchia of Music Time with Miss Cheryl LLC, the First Congregational Church of New Milford, the Marsh Parish House, and their representatives from any liability related to participation by myself, my child(ren), or caregiver(s).

Medical Consent:

I consent to emergency medical care for my child if I or designated caregiver cannot be reached. Music Time with Miss Cheryl LLC does not provide accident or hospitalization insurance; participants should have personal coverage and consider their own health, experience, and risk tolerance before participating.

Privacy / Data Collection

Music Time with Miss Cheryl LLC collects: child’s name and birth month & year, parent/guardian contact, payment info (via Stripe), and optional details (e.g., allergies). Used for registration, class updates, payments, and rosters. Stored securely, shared only for payments or as required by law. Parental consent is required for children under 13 (U.S.) or 16 (EU). Parents/guardians can review, update, or request deletion. Collection follows COPPA and GDPR rules.

Mailing List Acknowledgement

By registering for Lil' Music Makers programming, I acknowledge and agree that my name and email address will be added to the Music Time with Miss Cheryl mailing list for the purpose of receiving information regarding future programs, musical content, and related events. I understand that I may withdraw my consent and unsubscribe from these communications at any time by following the instructions provided in each email.
